Company Provided Benefits
Paid Time Off (PTO) blends personal time with vacation and holiday time for more flexibility in scheduling and using time off. PTO is accured based on hours paid with a minimum accrual of 19 days per year for less than 5.24 years of service and a maximum of 29 days per year after 14.24 years of employment. Time is accrued and posted quarterly.
Sick Time is accrued each quarter based on actual hours paid with a maximum accural of 10 days per year or 80 hours.
Bereavement Leave will be paid for up to three days (24 hours) in case of death of an immediate family member.
Group Long-Term Diability Insurance provides disability income to eligible employees, based on their monthly earnings. Eligibility means full-time employees regularly scheduled to work 60 or more hours bi-weekly.
Retirement Accumulation Plan (RAP) after completing one year of service (minimum of 1,000 hours worked) and for each qualifying year thereafter, Baptist Health Corbin will make an annual employer contribution on your behalf into your RAP account.
Educational Assistance Program will provide a portion of the cost of your tuition for classes related to your job or designed to prepare you for further opportunities within the system. Eligible classes can be taken at any accredited school, college or university that is approved by the Educational Assistance Committee. Tuition is available to all regular employees who have completed at least six continuous months of employment before class begins. Reimbursement is limited to $2,500 per fiscal year for full-time employees. Part-time employees will have reimbursement pro-rated, based on their full-time equivalent. Each course must be completed with a satisfactory grade to be reimbursed.

Family Medical Leave of Absence (FMLA) up to 12 weeks paid leave time possible for qualifying employees due to their own serious health condition, the birth or adoption of a child or the serious health condition of an immediate qualifying family member.
Employee Assistance Program (EAP) provides confidential help in coping with problems such as marital and family conflicts, depression, work-related stress, grief and loss, chemical dependency, or legal/financial difficulties with up to 10 free visits per year for employee or employee's household members.
Baptist Health Corbin Elected Benefits
Tiered Health Insurance Plans for employees regularly scheduled to work 17.5 hours per week. Baptist Health Corbin offers the Baptist Health Corbin-CHI Medical Plan, which is an PHO plan that covers a larger percentage of the cost for most services when you receive them from preferred providers who are a part of the plan's provider network.
Prescription Drug Benefit is a three-tier system where certain drugs are available from the Baptist Health Corbin pharmacy with no co-payment. This is available to any employee or qualifying dependent covered under the Baptist Health Corbin Health Plan.
Optional Dental Insurance available to employees regularly scheduled to work at least 17.50 hours per week. You have two dental plan options with four levels of coverage from which to choose. Along with comprehensive dental coverage, each dental option includes coverage for eye examinations, glasses and contact lenses through participating vision care professionals.
VisionCare Plan is a prepaid vision plan that lets you receive specified services from any vision care provider who participates in the VisionCare network. Employees must be regularly scheduled to work 17.50 hours every week to be elibible for coverage.
Dependent Life Insurance/Accidental Death/Dismemberment coverage may be purchased for your lawful spouse and dependent children based on your employment status.

Short-Term Disability Insurance available if you are a regular employee scheduled to regularly work 30 hours each week. Benefits begin after 14 days of absence and may be paid for up to six months due to an injury or illness that occurs while you're off-duty.
